The American Academy of Family Physicians suggests recording this information:
- Any illnesses or injuries you've had.
- Any times you were hospitalized.
- Any surgeries you've had.
- Anything you're allergic to.
- Illnesses affecting immediate family members.
- A list of all supplements, vitamins and medications you take, and when you take them.
