(HealthDay News) -- Keeping a health journal can help you better answer questions from your doctor.

The American Academy of Family Physicians suggests recording this information:

  • Any illnesses or injuries you've had.
  • Any times you were hospitalized.
  • Any surgeries you've had.
  • Anything you're allergic to.
  • Illnesses affecting immediate family members.
  • A list of all supplements, vitamins and medications you take, and when you take them.
